Welcome to Dynamic Mental Health Services, LLC (“we,” “our,” or “us”), a telehealth psychiatric service operated by Zinny Ezete, MSN, PMHNP-BC, licensed in New Jersey, New York, and Connecticut. These Terms of Use (“Terms”) govern your use of this website and all services provided through it. By accessing or using this website, you agree to be bound by these Terms and all applicable laws. If you do not agree, you must stop using the site immediately.
This site provides general informational content and allows scheduling of telehealth evaluations. It is not a substitute for professional medical advice, and use of the site does not establish a provider–patient relationship.
Services are provided exclusively by Zinny Ezete, MSN, PMHNP-BC, licensed in New Jersey, New York, and Connecticut. Telehealth services are available only if you are physically located in one of these states at the time of service.
This website and services are not intended for emergencies. If you or another person is in crisis, call 911 or go to the nearest emergency department. You may also contact the National Suicide & Crisis Lifeline by dialing 988.
While we aim to provide high-quality services, we do not guarantee specific results, diagnoses, or outcomes. All recommendations and documentation are subject to clinical discretion.
We may provide diagnostic or evaluation letters to support your school’s 504 or IEP process. Such letters do not guarantee approval of any accommodation. The school district retains final decision-making authority, and we bear no responsibility for district outcomes.
All services are cash-pay and must be prepaid. A superbill is provided for optional insurance reimbursement. Refund policy: Full refund for cancellations made 24+ hours in advance; 50% refund for cancellations between 4–24 hours; no refund for cancellations within 4 hours of the appointment.
We comply with the Health Insurance Portability and Accountability Act (HIPAA) and applicable state privacy laws. Telehealth sessions are conducted via SimplePractice, an encrypted, HIPAA-compliant platform. See our Notice of Privacy Practices for further details.

To the fullest extent allowed by law, Dynamic Mental Health Services, LLC, its owner, and affiliates shall not be liable for any direct, indirect, incidental, or punitive damages resulting from your use of this website or its services. We may update these Terms at any time by posting a revised version here. Upon posting, the revised Terms take effect immediately. Your continued use of the site constitutes acceptance of those changes.
These Terms are governed by the laws of the State of New Jersey and the Federal Arbitration Act (9 U.S.C. §§ 1–16), without regard to conflict of law principles. To the fullest extent permitted by law, any dispute, controversy, or claim arising out of or relating to these Terms or the services provided by Dynamic Mental Health Services, LLC shall be resolved exclusively through binding arbitration.
The arbitration shall be administered by the American Arbitration Association (AAA) under its Commercial Arbitration Rules then in effect, and conducted by a single arbitrator experienced in healthcare or professional services law. The arbitration shall take place in Essex County, New Jersey, unless otherwise agreed by both parties. Judgment upon the arbitration award may be entered in any court of competent jurisdiction.
Each party shall bear its own legal fees and costs unless otherwise required by law. This arbitration provision does not prevent you from filing a complaint with a state or federal agency such as the U.S. Department of Health and Human Services, Office for Civil Rights or any applicable state licensing board (NY OMH, NJ DCA, CT DPH), nor does it waive rights under state consumer protection statutes that prohibit mandatory arbitration.
If a court or regulatory body determines that arbitration is not permitted under applicable law, the parties agree that any such dispute shall be brought exclusively in the state or federal courts located in Bergen County, New Jersey.
By agreeing to these Terms, you acknowledge and agree that you are waiving the right to a trial in court or by jury, and that disputes will be resolved exclusively through binding arbitration.
